Please ensure you complete all required sections; otherwise, your form will be rejected. If an interpreter is required for the assessment process and delivery, please specify this on the form. Additionally, if a DASH form has been completed with the clients consent please send to enquiries@chab.org.uk with the person’s initials in the subject line.

We aim to respond to all enquiries within 48 hours. If information is missing from the form, this will delay the start of the programme.

We can only accept referrals for people who are not already on a programme and those who are assessed at standard to medium risk. Please contact us on 07428 581723 if you are unsure.
